RECREATIONAL ACTIVITY WAIVER AND RELEASE OF LIABILITY
ATV Extreme ATL | JLS Enterprises Group | JLS E GROUP INC | Cloverland ATL | Cloverland Ranch

1. Assumption of Risk
I understand and acknowledge that participating in activities such as ATV riding, horseback riding, laser tag,
and all other recreational events on the premises involves inherent risks, including but not limited to:
- Equipment failure
- Rough terrain and weather-related hazards
- Collisions with objects or other riders
- Animal encounters
- Physical strain, any injuries (including fractures, paralysis, or death)
- Delayed emergency care

I freely and voluntarily assume all risks, whether known or unknown, including those caused by ordinary
negligence of the operator or staff.

2. Release of Liability
In consideration of being allowed to participate, I release and hold harmless Cloverland ATL, ATV Extreme ATL, JLS E GROUP INC, JLS Enterprises Group, its owners, agents, employees, and affiliates (the "Released
Parties") from any and all claims, demands, or causes of action related to my participation, including claims of
negligence.

This release does not apply to gross negligence or willful misconduct.

3. Medical Treatment & Authorization
I understand there are no medical professionals on staff. I authorize staff to provide basic first aid and to
contact emergency services if needed. I agree not to hold the Released Parties liable for any assistance
provided in good faith.

4. Equipment Responsibility
I agree to operate all equipment responsibly and return it in the same condition. I am financially responsible
for any loss or damage caused by my misuse or negligence, up to $2,000. I authorize charges to my card after being notified of any damages after the incident.

5. Photo/Video Consent
I grant permission to use photos or video of me for marketing or promotional purposes without compensation.
I may revoke this permission by providing written notice prior to participation.

6. Rules & Conduct
I agree to follow all posted and verbal rules and staff instructions. Violating safety rules, removing helmets, harassing
animals, or behaving recklessly may result in expulsion without refund. Reentry after a ban is considered
trespassing.

7. Jurisdiction
This agreement is governed by the laws of Georgia, with an exclusive venue in Fayette County. If any part is
held unenforceable, the remainder will remain in effect. If legal action is required, Our company is entitled to recover legal fees and costs from the customer

8. For Parents/Guardians
I affirm that I am the legal parent or guardian of any listed minors. I consent to their participation and agree to
the terms of this waiver on their behalf.

9.Cancellation & Refund Policy Acknowledgment

By signing below, I acknowledge that I have read, understood, and agreed to JLS Enterprises group, Cloverland ATL, ATV Extreme ATL , JLS E GROUP INC Cancellation and Refund Policy. I understand that:

• All reservations must be canceled at least 48 hours after booking the appointment (not 48 hours before the appointment) to receive a full refund. You’re able to reschedule all the way up until the day before your appointment.

• Cancellations made after this time or failure to show up will result in forfeiture of any refund.

• I am not entitled to a refund after the cancellation window has passed, regardless of the reason.

• I agree not to initiate a credit card chargeback or payment dispute for any charges that fall within this policy.

• A copy of this signed agreement may be used as evidence in response to any such dispute.

BY SIGNING, I ACKNOWLEDGE THAT:
- I have read and understood this agreement.
- I voluntarily give up substantial legal rights.
